We have a convenient way of thanking you for being a great patient! Just join the smiling stars club!!

To become a club member all you need is a club card and an enthusiastic attitude about orthodontics !! Once you are a club member, you can start receiving tremendous prizes for following these simple rules:

• Have excellent oral hygiene
• Greatly improve hygiene since last visit
• Wear appliances and rubber bands successfully
• Visit your dentist for a cleaning and get your ‘Hygienist Card’ signed
• Have a positive attitude
• Bring in your report card and show off your great grades
• Visit our website and sign the guestbook
• Refer a friend for a fast 10 reward stamp!

Follow these rules and your patient manager will reward you by stamping your smiling stars club card. Of course all great efforts cannot be listed,but if we think you are doing a good job we will let you know!

So what do you do with the reward stamps? Trade them in for AWESOME PRIZES!!

• 1-10 = Anything in our prize chest (value of prizes listed in chest)
• 10 = A Ghosh T-shirt
• 20 = gift certificate to one of the following stores: Bath & Body Works, Borders, FYE, Friendly’s or
  Wal*Mart
  
  

• Remember: you MUST bring your card to every appointment!
